The assumptions are published too. Every default is stated with the reason for it — what is treated as recurring, what as one-off, what is not modelled at all.
Nothing is stored or transmitted. The calculations run entirely in your browser. You can verify that in your browser's network inspector: use any tool and watch that no request leaves the page.
No health data is collected. A publication covering weight-management medicine has no business holding readers' body weights.
The page works without JavaScript. The explanation is readable even where the calculator is not.
What none of them will do
Tell you which provider to use. Tell you what dose to take. Tell you whether a medication is appropriate for you, or whether you meet eligibility criteria. Those are clinical decisions belonging to a licensed clinician who knows your history — and a calculator that implied otherwise would be doing something worse than being unhelpful.
