Tirzepatide Cost Without Insurance
Without insurance, the cheapest FDA-approved route is $149 a month for oral orforglipron or oral Wegovy tablets at starting doses. Brand Zepbound is $299 direct from LillyDirect. Compounded injectable tirzepatide runs $169 to $399. Sixteen compounded offerings in our dataset cost more than buying the approved drug direct.
Start with the manufacturer, not a platform
The single most expensive mistake in self-pay GLP-1 purchasing is buying a brand-name drug through a telehealth platform that resells it near retail. Our dataset contains brand Wegovy at $1,794 a month all-in from one provider and brand Mounjaro at $2,048 from another. NovoCare sells Wegovy direct for $349. Lilly runs no self-pay programme for Mounjaro, but Zepbound is the same molecule at $299 to $449.
That is a four to twelvefold difference for the identical medicine, and it is disclosed pricing rather than fraud. Price any brand drug at LillyDirect or NovoCare before buying it anywhere else.
The self-pay ladder in 2026

Where self-pay logic breaks down
The overlap in that ladder is the finding. A compounded programme at $349 costs more than the approved injectable at $299 and more than twice the approved oral. Whatever the argument for compounded medicine is at that price, it is not a cost argument.
The cost argument holds firmly at the bottom of the range, and it holds for flat-rate programmes at maintenance doses, where a dose-independent price beats a brand tier that rises to $449.
Before you conclude you have no coverage
Three checks are worth the twenty minutes, because any of them beats every self-pay figure above.
- Check your plan's formulary directly for Zepbound and Wegovy, and check whether the exclusion is for weight management specifically. Coverage for a diabetes indication is often separate.
- Ask about prior authorisation criteria. Many plans cover with documentation of BMI and comorbidities.
- Check manufacturer savings programmes, which are separate from the self-pay pricing above and have their own eligibility rules.
Where a commercial plan covers either brand, the cost can fall to roughly $25 a month. No self-pay route competes with that, and no compounded programme comes close.
