Microdose Tirzepatide: Evidence, Cost and Limitations

What you are actually buying

A microdose programme prescribes well below the doses used in the pivotal trials. Typical programmes deliver around 1 mg a week. SURMOUNT-1 used 5, 10 and 15 mg, and reported mean reductions of roughly 15.0%, 19.5% and 20.9% respectively over 72 weeks against 3.1% with placebo.

The dose-response relationship is the whole story. If effect rises from 5 to 15 mg, a dose below 5 mg cannot be assumed to match any of those figures. No trial has published what 1 mg does.

The regulatory mechanics, stated plainly

Compounding a copy of an available approved product is generally not permitted. Compounding something a prescriber documents as clinically different for an individual patient is. "Personalised dosing" is the mechanism by which that documentation happens, which is why the microdose category expanded when the shortage listings ended rather than when new evidence appeared.

Where a lower dose is legitimate

Prescribers reduce doses for real clinical reasons: side-effect burden, comorbidities, patient preference, or maintenance after a period at full dose. A clinician choosing a lower dose for a specific patient is practising medicine, and a patient who cannot tolerate 10 mg is better served at 5 than at nothing.
