Microdose Tirzepatide: Evidence, Cost and Limitations
Microdose tirzepatide programmes deliver roughly 1 mg weekly against the 5, 10 and 15 mg studied in SURMOUNT-1, where the effect rose with dose. No trial has measured a microdose. They cost $147 to $349 a month, and they exist partly as a regulatory route: documenting a clinical difference for an individual patient is one of the surviving bases for compounding.
What you are actually buying
A microdose programme prescribes well below the doses used in the pivotal trials. Typical programmes deliver around 1 mg a week. SURMOUNT-1 used 5, 10 and 15 mg, and reported mean reductions of roughly 15.0%, 19.5% and 20.9% respectively over 72 weeks against 3.1% with placebo.
The dose-response relationship is the whole story. If effect rises from 5 to 15 mg, a dose below 5 mg cannot be assumed to match any of those figures. No trial has published what 1 mg does.
The regulatory mechanics, stated plainly
Compounding a copy of an available approved product is generally not permitted. Compounding something a prescriber documents as clinically different for an individual patient is. "Personalised dosing" is the mechanism by which that documentation happens, which is why the microdose category expanded when the shortage listings ended rather than when new evidence appeared.
That is not an accusation of bad faith. It is the regulatory context, and it explains the timing better than a clinical rationale does.
The pricing
NexLife (opens nexlife.us in a new tab) at $147 on a twelve-month plan and $189 month-to-month, verified. Enhance.MD at $169 for a stated 1 mg weekly programme including lab testing, delivered twelve-weekly. Shed at $199 with a two-month minimum. bmiMD at $349, which is above most competitors' standard injectable pricing.
For context, the cheapest FDA-approved GLP-1 is $149 a month — orforglipron or oral Wegovy tablets at starting doses. Two microdose programmes cost more than an approved product with published trial data.
Where a lower dose is legitimate
Prescribers reduce doses for real clinical reasons: side-effect burden, comorbidities, patient preference, or maintenance after a period at full dose. A clinician choosing a lower dose for a specific patient is practising medicine, and a patient who cannot tolerate 10 mg is better served at 5 than at nothing.
That differs from a programme built around a low dose as its commercial entry tier and marketed beside figures collected at ten to fifteen times the dose.
Four questions before enrolling
- What weekly dose does this deliver, in milligrams?
- What evidence exists for that dose specifically?
- Is the plan to escalate, and does the price change if I do?
- If this is maintenance dosing, what was I meant to have done first?

Questions readers actually ask
Does microdose tirzepatide work?
No trial has measured a roughly 1 mg weekly dose over a trial-length period. The dose-response data suggests a smaller effect than the studied doses produced.
Why do microdose programmes exist?
Partly clinical — lower doses suit some patients — and partly regulatory: documenting a clinical difference for an individual patient is one of the surviving routes for compounding when an approved product is available.
