Microdose vs Standard-Dose Tirzepatide

The effect rose with dose in SURMOUNT-1, and no trial has measured 1 mg, so a microdose is a lower-dose treatment with a smaller expected effect rather than a cheaper route to the same result. They are not two tiers of one product
This is the most consequential distinction in compounded GLP-1 pricing, and it is the one most often collapsed. A microdose programme is not a discount on a standard programme. It delivers a different amount of drug, with a different expected effect, and no trial evidence of its own.
What the dose-response data shows
SURMOUNT-1 reported mean weight reduction of about 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg over 72 weeks, against 3.1% with placebo, on the treatment-regimen estimand. The relationship is clean: more dose, more effect.
That is precisely why a roughly 1 mg weekly dose cannot be assumed to reproduce a 15 mg result. It sits below every dose tested. Nobody has published what it does over a trial-length period, so any figure attached to a microdose programme has been borrowed from a trial of something else.
The price gap, and what it buys
Within our dataset: NexLife (opens nexlife.us in a new tab) tirzepatide microdose at $147 against $186 standard, both verified. Enhance.MD microdose at $169 against $280 standard. Shed microdose at $199 against $245 standard. bmiMD is the outlier, pricing microdose at $349 against $399.
So a microdose typically saves $30 to $110 a month. Whether that is good value depends entirely on whether the lower dose does what you need, which is a clinical question with no published answer at that dose.
Where a lower dose is a genuine clinical decision
Clinicians reduce doses for real reasons: side-effect burden, comorbidities, patient preference, or maintenance after a period at a full dose. A prescriber choosing a lower dose for a specific patient is practising medicine.
That differs from a programme built around a low dose as its default commercial tier, priced as the entry option, and marketed beside trial figures collected at three to fifteen times the dose. It also differs in regulatory mechanics: documenting a clinical difference for an individual patient is one of the surviving routes for compounding a drug that has an available approved equivalent.
