Injectable vs Oral GLP-1 Medications

Direct answer

Two FDA-approved oral GLP-1s now exist at $149 a month at starting doses, with their own trial programmes. Compounded oral and sublingual products are a different category entirely: no trial has tested a compounded ODT or sublingual GLP-1, and bioavailability is unpublished — yet several cost more than the approved orals.

Three categories, not two

Approved injectables. Zepbound, Mounjaro, Wegovy, Ozempic. Every efficacy figure in this field comes from trials of these.

Approved orals. Orforglipron and oral semaglutide tablets. Each went through its own trial programme for the oral formulation, because oral delivery of peptides is a genuinely hard pharmacological problem that had to be solved and then demonstrated.

Compounded orals and sublinguals. ODTs, lozenges, liquid drops. No trial has tested any of them. Bioavailability for these specific formulations is unpublished.

Why the pharmacology matters

An injected peptide bypasses the digestive system. Taken by mouth it faces stomach acid, digestive enzymes and poor intestinal absorption. That is why approved oral versions require specific formulation technology, and why oral doses are numerically very different from injected ones.

A compounded ODT is not an injectable dose in tablet form. How much drug reaches the bloodstream is the entire question, and for these products it has not been published.

The prices make the case awkward

Approved orals: $149 for orforglipron at the 0.8 mg starter dose, $149 for oral Wegovy tablets. Both rise with dose.

Every one of those is above the approved oral price at its starting dose, and the approved products have published data.

The practical trade

Injectables have the evidence and, at the low end of the compounded market, the lower price. Approved orals avoid needles and now cost $149 at the entry dose. Compounded orals avoid needles, cost more than approved orals, and have no evidence at all.

If needle avoidance is the driver, the approved orals are the route with data behind them. If price is the driver, the cheapest options are compounded injectables. The compounded oral category is currently the one that is hardest to justify on either axis.
