Tirzepatide and Chronic Kidney Disease

The honest position

There is no completed, published, dedicated kidney outcomes trial for tirzepatide. It is listed among the indications under evaluation alongside heart failure with preserved ejection fraction, obstructive sleep apnoea and morbidity and mortality in obesity — two of which have since produced results, and this one has not.

That is the whole substantive answer, and this page exists mainly to say it clearly, because the absence is routinely filled with evidence borrowed from a different drug.

Where the class-level kidney evidence actually comes from

FLOW, which studied semaglutide in people with type 2 diabetes and chronic kidney disease. That trial is the reference point for GLP-1 kidney outcomes, and it is a semaglutide trial.

Tirzepatide adds GIP receptor agonism to semaglutide's GLP-1 mechanism. That difference produced materially different weight outcomes in SURMOUNT-5 — 20.2% against 13.7%. It is not safe to assume the difference runs only in one direction, or that kidney effects transfer unchanged.

Why the mechanism makes benefit plausible

Chronic kidney disease in this population is driven substantially by diabetes, hypertension and obesity. A drug that improves glycaemic control, reduces weight and lowers blood pressure has several plausible routes to slowing progression, and SUMMIT reported reduced systemic inflammation as well.

Plausible is not demonstrated. EVOKE is the cautionary example in this class: a well-powered trial of oral semaglutide in Alzheimer's disease, built on a plausible mechanism and an observational signal, returned a clean null over two years in 3,808 participants.

What to be careful of

Marketing that cites "kidney protection" for tirzepatide without naming a trial. Ask which study, in which population, with what endpoint. If the answer is FLOW, that is semaglutide. If there is no answer, there is no trial.

The same caution applies to compounded preparations, which appear in no kidney trial of any molecule.

What would change this page

Publication of a dedicated tirzepatide kidney outcomes trial, or a prespecified kidney analysis from an existing programme. At that point we would record design, population, dose, comparator, duration, endpoint, results with the estimand named, adverse events and discontinuations, with identifiers, as for every other trial here.

Until then this page stays short, because there is not more to say honestly.
