Cold-Chain Shipping for Compounded GLP-1s
GLP-1 medications generally require refrigeration, and compounded preparations ship from pharmacies to homes rather than through pharmaceutical distribution. Establish before ordering what the storage requirement is, what packaging is used, and what happens if a shipment arrives warm — get the replacement policy in writing.
Why this is a real risk rather than a theoretical one
Peptide medications are temperature-sensitive. Approved GLP-1 products carry refrigerated storage requirements with defined room-temperature allowances, and compounded preparations generally carry similar or stricter conditions.
A shipment travelling to a residential address in summer, sitting on a doorstep for several hours, is a different environment from a pharmacy-to-pharmacy cold chain. The risk is not that the medicine becomes dangerous; it is that potency degrades in ways the patient cannot detect.
What to establish before your first order
- The storage requirement for your specific preparation, including any permitted room-temperature period and whether it is cumulative.
- The packaging. Insulated container, coolant type, and whether a temperature indicator is included.
- Delivery timing and signature. Whether you can schedule delivery, and whether a signature is required rather than a doorstep drop.
- The replacement policy. What happens if the package arrives warm, the coolant is spent, or delivery is delayed — and whether replacement is at the provider's cost.
- Who to contact, and how quickly they respond, when a shipment arrives in doubtful condition.
Practical handling once it arrives
Open the shipment promptly rather than leaving it. Check any temperature indicator. Refrigerate immediately unless the labelling says otherwise. Note the beyond-use date, which for a compounded sterile preparation may be considerably shorter than an approved product's expiry.
If a shipment arrives warm, photograph the packaging and the indicator before storing anything, and contact the provider the same day. A photograph taken at the point of receipt is the evidence a replacement claim rests on.
The multi-month supply question
Some programmes ship on twelve-weekly or longer cycles, which reduces the number of cold-chain events and increases the amount at risk in each one. It also means the beyond-use date has to cover the whole period.
Ask directly: does the beyond-use date on my supply cover the interval until my next delivery? If it does not, the shipping schedule and the product's stability are not aligned.

- FDA registered outsourcing facilitiesConfirms a facility is currently registered under 503B, and links the searchable product report database. Registration is not FDA approval and is not evidence of CGMP compliance.
- FDA outsourcing facility registration Q&AThe agency's own explanation of exactly what registration does and does not mean. Worth reading before accepting any provider's characterisation of its pharmacy.
- NABP directory of state boards of pharmacyRoutes you to the board that licenses a pharmacy, and to its licensee and nonresident registers. Check the licence is current, not merely present.
- FSMB directory of state medical boardsRoutes you to the board that licenses a prescriber, and to its public licence lookup. An NPI number is identity in a national registry, not a licence.
Every one of these is the register held by the body that issues the credential. A claim checked against the page that made it has not been checked.
| Claim | Proves | Does not prove |
|---|---|---|
| FDA-registered outsourcing facility | A registration was filed | That any product is FDA-approved |
| State pharmacy licence | A board issued a licence | That it is current — check it |
| Accreditation | A voluntary assessment was passed | Batch quality or product approval |
| USP <797> compliance | A sterile compounding standard is claimed | That it was independently audited |
| Third-party tested | Something was tested | What, by whom, or how often, without a certificate |
| NPI number | Identity in a national registry | A current licence in any state |
| FDA-approved compounded medicine | Nothing — the category does not exist | Treat as a red flag |
Providers publishing terms openly are easier to hold to a written answer. NexLife states its pricing and dose-coverage on its own plan pages before intake, which is why its records are the six we marked verified — though it has not named its fulfilling pharmacy to us either. Its published plans.
Questions readers actually ask
What if my medication arrives warm?
Photograph the packaging and any temperature indicator before storing it, and contact the provider the same day. Establish the replacement policy before ordering.
Does a longer shipping interval matter?
It reduces the number of cold-chain events and increases what is at risk in each. Check that the beyond-use date covers the interval to your next delivery.
