The label change

Zepbound is now indicated both to reduce excess body weight and maintain reduction long term in adults with obesity or overweight with a weight-related comorbidity, and to treat moderate-to-severe obstructive sleep apnoea in adults with obesity.

That followed the SURMOUNT-OSA programme: two phase 3, randomised, double-blind, placebo-controlled trials under a single master protocol, one enrolling participants not using positive airway pressure and one enrolling those who were, with tirzepatide at maximum tolerated dose or placebo for 52 weeks.

What the trials found

Tirzepatide reduced the apnoea-hypopnoea index — the count of breathing interruptions per hour that defines severity — alongside body weight, hypoxic burden, high-sensitivity CRP and systolic blood pressure, and improved sleep-related patient-reported outcomes.

A prespecified secondary analysis published in Nature Medicine in January 2026 found improvement in cardiometabolic risk markers, with mediation analysis suggesting that treating both the sleep-disordered breathing and the obesity is likely required to optimise cardiometabolic benefit.

The coverage implication most coverage missed

Many insurance plans exclude weight-management indications while covering the same molecules for other purposes. An on-label indication for obstructive sleep apnoea gives an insurer a different question to evaluate.

For a patient with both obesity and diagnosed moderate-to-severe OSA whose plan excludes weight management, that is potentially the difference between a $25 copay and $449 a month. It is worth raising specifically with the prescriber and the plan.

What it does not mean

That tirzepatide replaces PAP therapy. Reduction in apnoea-hypopnoea index is a clinical improvement, not resolution, and in the second study PAP was withdrawn before assessment — a design choice for isolating drug effect, not a test of substitution.
