What was announced
On 6 February 2026 the FDA announced it would take steps to restrict GLP-1 active pharmaceutical ingredients that continue to be used in non-FDA-approved compounded products, and to address misleading direct-to-consumer advertising.
Framed at the time as an escalation, it reads in retrospect as the announcement of a strategy. The 503B bulks exclusion proposal followed on 30 April.
Why the shift mattered
Enforcement to that point had been letter-based. More than 55 warning letters went to online sellers in September 2025, and more than 40 to telehealth-based compounding pharmacies later that year. Compliance observers described the effect as limited — providers largely continued advertising and compounding, reframing products as patient-specific variants.
Letters address individual actors one at a time. Restricting the active ingredient addresses the supply. That is a structural intervention rather than an enforcement one, and it is what the bulks proposal delivers.
The sequence, assembled
- October 2024 — tirzepatide shortage determined resolved, then remanded for reevaluation amid litigation.
- February 2025 — semaglutide shortage resolved.
- March 2025 — court denies preliminary injunction in Outsourcing Facilities Association v. FDA.
- May 2025 — enforcement discretion window for 503B semaglutide closes.
- September 2025 — 55+ warning letters.
- Late 2025 — 40+ further letters.
- February 2026 — API restriction announced; TrumpRx launches the day before.
- Early 2026 — 30 more letters, with explicit legal threat.
- April 2026 — 503B bulks exclusion proposed.
- June 2026 — 25 more letters; comment period extended.
Nothing in that list is ambiguous about direction.
What it means for a patient reading provider marketing
Providers describing the regulatory position as unsettled, or the compounded market as continuing indefinitely, are describing a position the agency has been publicly closing for two years.
That does not make compounded medicine unlawful today, and it does not make a provider dishonest for supplying it. It does mean a twelve-month prepayment is a bet on a regulatory outcome, and a provider who will not say so is selling certainty they do not have.
