Tirzepatide vs Semaglutide

Direct answer

SURMOUNT-5 compared them head to head over 72 weeks in 751 adults with obesity and without diabetes: −20.2% mean weight change with tirzepatide against −13.7% with semaglutide, a difference of about 7.8 kg. Tirzepatide costs more in nearly every programme — typically $40 to $80 a month more for compounded, and $299 against $149 for the cheapest approved routes.

The mechanism, briefly

Semaglutide activates the GLP-1 receptor. Tirzepatide activates both GLP-1 and GIP. The dual-agonist hypothesis is that GIP receptor expression in appetite-regulating brain regions does not fully overlap with GLP-1 expression, so acting on both produces more effect than either alone. SURMOUNT-5 is the trial that tested it directly.

What the head-to-head found

Least-squares mean weight change at 72 weeks was −20.2% with tirzepatide and −13.7% with semaglutide, both at maximum tolerated dose. In absolute terms −22.8 kg against −15.0 kg. Waist circumference fell 18.4 cm against 13.0 cm. More participants on tirzepatide reached every threshold from 10% upward.

Serious adverse events occurred in 4.8% of the tirzepatide group and 3.5% of the semaglutide group. Six participants in each group discontinued because of adverse events. Gastrointestinal effects dominated in both.

Three caveats belong with those numbers. The trial was open-label. It compared maximum tolerated doses rather than fixed doses, so it tested two titration strategies. And it was funded by the company that makes the winning drug — which does not make the result wrong, but belongs in the citation.

The cost difference, in both markets

Compounded. Tirzepatide is priced above semaglutide almost everywhere: $186 against $145 at NexLife (opens nexlife.us in a new tab), $199 against $133 at Oak Longevity, $278 against $178 at Mochi Health, $399 against $289 at bmiMD. Found is the exception, charging $169 for both.

Approved. The cheapest approved semaglutide route is oral Wegovy tablets at $149 direct from NovoCare. The cheapest approved tirzepatide is Zepbound at $299 direct from LillyDirect. Orforglipron, an approved oral GLP-1 that is neither molecule, also starts at $149.

How to think about the trade

Tirzepatide produced roughly a third more weight reduction in the one trial that compared them, at a cost premium that varies from nothing to double depending on route. Whether that trade is worth making is not a question a comparison page can answer — it depends on your tolerability, your budget over an indefinite period, and what your clinician advises given your history.

What is worth stating plainly: the trial compared FDA-approved subcutaneous injections at studied doses. It says nothing about compounded preparations of either molecule, about microdoses of either, or about oral and sublingual compounded formats. Those results do not transfer.
